Form Block
Create a Form block inside Learn content so learners can submit structured responses. You can add fields, set which ones are required, and add close reasons for reviewers.
Note: The Form block requires the Contribute feature to be enabled on your tenant. If you do not see Form as a block option when editing Learn content, ask the GFoundry team to enable it.
How do I add a Form block to Learn content?
Go to Modules > Learn > Learn Content, then open the Learn Content item you want to edit.
In the content blocks area, select Form.
Choose Create.
If you are creating content from the wizard, you can also start from Generate Content with AI or Add Content Manually, then add a Form block later in the content editor.
How do I add fields to the form?
Go to Modules > Learn > Learn Content, open the Learn Content item, and then open the Form block.
In Form Fields, select Add Field.
Fill in Field name.
Choose the Field type.
Turn Is required? on if the field must be filled in before submission.
If needed, set Max characters.
Select Create or Update.
For multiple-choice fields, use Add Choice to add answers. For Likert fields, use Add Choice and Add Question to build the scale.
How do I add close reasons?
Go to Modules > Learn > Learn Content, open the Learn Content item, and then open the Form block.
Go to Close Reasons.
Select Add Close Reason.
Enter the reason text and save it.
Close reasons give reviewers ready-made options when they close submissions.
How do I remove the Form block?
Go to Modules > Learn > Learn Content, open the Learn Content item, and then open the Form block.
Select Manage Form.
Choose Remove form?.
Confirm the removal.
Removing the form also removes its fields from the Learn content.
