Reference: supported form field types
The field types you can add to a form, as they appear in the Field type list, and the extra settings some of them need.
What form field types can I add?
When you add or edit a field on the Form Details page, the Field type list offers the types below. Every type has a name, an optional description and the Is required? option.
Text Field: free text. You can set Max characters (120 if left empty, up to 5000).
Single Choice Field: the person selects one option. Needs at least one choice.
Multiple Choice Field: the person can select several options. Needs at least one choice.
Date Field: a date.
GPS Coordinates Field: a location.
Upload Image Field: the person uploads an image.
Upload PDF Field: the person uploads a PDF file.
eNPS Field: one rating on a 10-point scale.
Likert Field: a set of Questions that share the same answer scale of Choices. Each question is answered with one choice. Needs at least one choice and one question.
In the Form Fields table the types are shown in short form: Text, Single Choice, Multiple Choice, Date, GPS, Photo, PDF, eNPS and Likert.
What does Is required? do?
When a field is required, the form can only be submitted if that field is filled in. If a field is not required, people can submit the form without completing it. The Form Fields table labels each field Required or Optional.
How are choices managed?
For Single Choice Field and Multiple Choice Field, each choice has a Name, which people see, and an optional Value, which is not shown to them. When values are numeric, the values of the selected choices are added up into the Score column of the Excel export.
For a Likert Field, you manage Choices (each with a Choice name) and Questions (each with a Question name) separately. In the Excel export, each question gets its own column.
Can I change a field's type later?
Yes, with Change type, but the field is replaced by a new question. Existing answers stay with the original question. See How to add and configure form fields.
