How do I add a form field?

  1. Go to Modules > Forms & Surveys > Forms List and open the form.

  2. On the Form Details page, in the Form Fields section, click Add Form Field.

  3. Enter the Field name and, if you want, a Description.

  4. Choose the Field type.

  5. Select Is required? if the field must be filled in.

  6. Click Create.

To change a field later, click it (or its edit icon) in Form Fields, make your changes and click Update. To change the order of the fields, drag them by the handle on the left of each row.

Can I change a field's type?

Yes, but it replaces the question. On an existing field the Field type is locked: click Change type, pick the new type and confirm in Change the question type?. The window says how many responses the form already has and how many options the new type discards.

Answers already submitted stay with the original question and keep appearing in the reports, but are not linked to the new question.

What can I configure in a field?

Every field has a name, an optional description, a type and the Is required? option. When a field is required, the form can only be submitted if that field is filled in. The Form Fields table shows each field as Required or Optional.

For a Text Field, you can also set Max characters. If you leave it empty, the limit is 120 characters. The maximum you can set is 5000.

How do I add choices to choice fields?

For Single Choice Field and Multiple Choice Field, add at least one choice before saving ("Please provide at least one choice.").

  1. In the Choices section of the field, click Add Choice.

  2. Enter the Name people will see and, optionally, a Value.

  3. Click Create, and repeat for each option.

The Value is not shown to people filling in the form. When choices have numeric values, the values of the selected choices are added up into the Score column of the Excel export. You can drag choices to reorder them, and edit or delete them from their row.

How do I configure Likert fields?

A Likert Field has two parts: Choices (the answer scale) and Questions (the statements to rate). It needs at least one of each.

  1. Choose Likert Field as the Field type.

  2. In Choices, click Add Choice, enter the Choice name and click Create. Repeat for each point of the scale.

  3. In Questions, click Add Question, enter the Question name and click Create. Repeat for each question.

  4. Save the field.

Each question is answered with one of the choices. You can reorder, edit or delete choices and questions from their rows.

Can I change the type of an existing field?

Yes, but it replaces the question. On an existing field the type is locked ("Changing the type replaces this question."); click Change type to unlock it. When you save, you are asked to confirm. Answers already given stay with the original question and keep appearing in the reports, but are not linked to the new question. If the new type cannot keep the original options (for example, changing a choice field to a text field), those options are deleted.

How do I delete a field?

Click the delete icon on the field's row in Form Fields, or open the field and click Delete, then confirm.
