Make a form anonymous so responses are not shown with the respondent's name, or limit each user to one response. Both are checkboxes in the form settings and can be used together.

On the Form Details page, an anonymous form shows the notice "The responses to this form are anonymous". In the Dashboard, each form is labelled Anonymous or Identified.

For an anonymous form, the respondent is not named in the Responses List, in Response Details, in the individual response PDF, or in the User and Email columns of the Excel export.

The Excel export (Export Report or Export Responses) still fills in the respondent's Team column, and the Cost Center column when your organisation uses cost centers. If a team is small, answers may still be traceable to a person. Take this into account before you promise full anonymity.

People filling in an anonymous form in the platform see the tag "Anonymous form" and the note "This form is anonymous".

When you select Single submission per user, each user can only submit one response to that form. Use it when you need one answer per person, such as a survey or an evaluation.

Yes. The two checkboxes are independent, so you can enable both on the same form. Anonymous controls whether the respondent is named; single submission controls how many responses each user can send.

Yes. In the form settings, use Users to be notified on form submission to choose the users who receive a notification when the form is submitted. The selected users are listed as Notification Users on the Form Details page.

Forms created from the Form block of a Learn Content show the notice "This form is managed by a Learn Content. Some settings are controlled by the Learn module and cannot be changed here." Is Anonymous?, Single submission per user and the notification users are still available in the form settings, while options such as Unlisted form, Is Learn Evaluation Form?, segmentation and status are hidden, and the form cannot be deleted from Forms & Surveys.
