How Learn Content-managed forms work
A Learn Content-managed form is created by the Form block of a Learn Content and belongs to it. You can see it and manage its fields in Forms & Surveys, but some settings are controlled by Learn and the form cannot be deleted there. This is different from the "Is Learn Evaluation Form?" option.
What is a Learn Content-managed form?
When you add a Form block to a Learn Content (in the Learn module), the platform creates a form linked to that Learn Content, so learners can submit structured responses inside the content. The Form block is only available when Forms & Surveys is enabled for your organisation. See Form Block.
In Modules > Forms & Surveys > Forms List, these forms carry a Learn Content label with a link to the Learn Content. If that Learn Content no longer exists, you see "The associated learn content is no longer available."
What can I change in Forms & Surveys?
Opening the form shows the notice "This form is managed by a Learn Content. Some settings are controlled by the Learn module and cannot be changed here." On its Form Details page you can:
Add, edit, reorder and delete form fields in Form Fields.
Add, edit and delete close reasons in Close Reasons.
Click Edit to change the name, description, image, Is Anonymous?, Single submission per user and Users to be notified on form submission.
For these forms, Is Learn Evaluation Form?, Unlisted form, Groups Segmentation, Team Segmentation and Status are not shown.
Can I delete a Learn Content-managed form?
Not from Forms & Surveys. The Delete action is disabled on the Form Details page ("This form is managed by a Learn Content and cannot be deleted here.") and is not offered in the Forms List. To remove the form, remove it from the Form block of the Learn Content that owns it.
How is this different from "Is Learn Evaluation Form?"
Is Learn Evaluation Form? is a checkbox on a regular form, not a Learn Content-managed one. Its help text reads "Enable this to use this form as an evaluation form in the Learn module." When it is selected, the form can be chosen as the Form resource of an evaluation step (for example a Satisfaction Survey) in a Training Program or Curriculum. Forms without this option do not appear in that selector.
To create such a form:
Go to Modules > Forms & Surveys > Forms List.
Click New Form.
Enter the Form name.
Under General, select Is Learn Evaluation Form?.
Click Create, then add the fields on the Form Details page.
A form created this way is a regular form: you can edit all its settings and delete it from Forms & Surveys.
