Follow this troubleshooting flow

  1. Identify whether the user is looking for Feedback, a form or survey, or a questionnaire inside another process.

  2. Confirm that the relevant module or feature is enabled for the organisation.

  3. Check that the intended questionnaire, template, feedback scope, or form exists and is enabled or published.

  4. Review its opening and closing dates and confirm that the user is checking it during the available period.

  5. Confirm the audience, groups, organisational scope, role, and any permission rule that controls access.

  6. Check how it should be accessed. An unlisted form may be available only through its direct link and may not appear in the forms list.

  7. Review restrictions such as read-only mode, one response per user, or a previous submission.

  8. After an authorised correction, ask the affected user to start a new session and verify the expected access route.

What should I check for Feedback?

Confirm that the feedback scope is enabled, uses the intended context, contains the required questions, and allows the affected role to submit feedback. A disabled or read-only scope can remain visible while preventing new submissions.
