How do I export the responses of one form from the Dashboard?

  1. Go to Modules > Forms & Surveys > Dashboard.

  2. In the Forms List table, find the form.

  3. Open the actions menu for that form.

  4. Click Export Responses. The Excel file is downloaded in a new tab.

Export Responses only appears for forms that have at least one response.

How do I export from the Responses List?

  1. Go to Modules > Forms & Surveys > Responses List.

  2. If you want, filter by form (Select Form) and by status.

  3. Open the Export menu at the top of the list.

  4. Click Export Report. The Excel file is downloaded in a new tab.

The export uses the form and status filters currently applied. Without filters, it includes the responses of all forms.

What is included in the Excel export?

The file has one sheet per form. Each row is a response, with these columns:

  • Created, User, Email, Team, and Cost Center when your organisation uses cost centers.

  • Status. Responses still under review appear as "Created".

  • Score: the sum of the values of the selected choices, when choices have numeric values.

  • One column per form field. A Likert Field gets one column per question ("Field name - Question"). Image and PDF uploads appear as links.

  • Notes: the internal notes added to the response.

For anonymous forms, User and Email do not identify the respondent, but the Team and Cost Center columns are still filled in.

Can I export a single response?

Yes. In the Responses List, open the row's actions menu and click Download PDF, or click Download PDF on the Response Details page. The PDF contains the full form with the response's answers.

When the response belongs to a Learn assessment, the PDF also says where it comes from (training program, curriculum or Learn Content) and shows the Evaluation Score and Result (Passed or Not passed) recorded in the evaluation. Until the evaluation is scored, those lines are not printed.
